NTSB 143 page report on Organizational Factors in Metro-North Railroad Accidents

Summary

During the time period between May 2013 and March 2014 the National Transportation Safety Board (NTSB) launched investigative teams to five significant accidents on the Metro-North Railroad (Metro-North): 

(1) the May 17, 2013, derailment and subsequent collision in Bridgeport, Connecticut; 

(2) the May 28, 2013, employee fatality in West Haven, Connecticut; 

(3) the July 18, 2013, CSX drailment on Metro-North tracks in The Bronx, New York; 

(4) the December 1, 2013, derailment in The Bronx, New York; and 

(5) the March 10, 2014, employee fatality in Manhattan, New York. In combination, these accidents resulted in 6 fatalities, 126 injuries, and more than $28 million in damages.

This special investigation report discusses all five of the recent Metro-North accidents investigated by the NTSB, examines some of the common elements of these accidents, and addresses the steps that Metro-North, the Metropolitan Transit Authority (MTA), and the Federal Railroad Administration have taken as a result of these investigations. This report also highlights lessons learned and provides recommendations to Metro-North, MTA, and several other entities to improve railroad safety on Metro-North and elsewhere.

Today's board meeting reveal:
 
MTA and third party contractor "STV Inc." is doing an engineer study on the platform screen doors. The contract awarded in September for $25 million ( which they also doing engineering Sandy fortify projects )
 
There are lots of complex engineering issues like the car fleet, station layout, spacing, clearance, timing, power, ventilation, maintenance, etc.
 
They will demo the PSDs on one station.
 
To sum up... there's "not one solution." "

I just remembered, that the (H) was an 8 Aveune Line and had good service and the (K) was a line that substituted for the © to World Trade Center and for (E) train customers going north on the © Line. Now which line is better?

Just like the express forum that I made, which line is the best local? I'll start by saying that the (6) is the best local, on the weekends, because it has a higher frequency and has more trains coming in each direction during midday.

A brutal video catches an NYPD officer bash an alleged fare beater in the head with a baton, opening a gash in the man’s head and sending him stumbling around the Brooklyn subway station in a daze.
 
Blood spurted from the cut and sprayed witnesses and the walls at the Myrtle Avenue (J)(M)(Z) stop during the arrest Thursday just before 8 p.m., Marielle Anas told the Daily News.
 
 
“It was one of the most awful things I’ve ever seen,” Anas, 28, said. “I have blood all over my jeans. The brutality of the nightstick – it was crazy. It looked like his head exploded. I couldn’t believe I was seeing it happen.”
 
Anas was on her way home from work when she stumbled upon the scene in the mezzanine area of the Bushwick station. The man was sitting on a wooden bench as the officer towered over the alleged fare beater and threw at least three punches at the baby-faced man’s head.
 
“The kid wasn’t doing anything – that’s why I started recording,” Anas said. “If he can't subdue this kid – he wasn’t even resisting that much!”
 
As Anas began filming, other bystanders, including the man’s girlfriend, began to yell at the officer to stop the beatdown. Anas went to call 911 just as the officer pulled out his truncheon and cracked the man on the head, sending him reeling across the room in a daze as blood poured from his skull.
 
Another clip, first obtained by PIX 11, captures the officer brutally arresting the man, identified as 20-year-old Donovan Lawson, and using the nightstick across his head, reportedly in violation of New York Police Department policy.
 
“Everyone was horrified,” Anas said. “There were at least 30 or 40 people that gathered. It just seemed so obvious that it was unnecessary violence.”
 
The NYPD Internal Affairs Unit is investigating the incident, according to PIX 11. Lawson has no prior arrest, a source told the station, but was arrested for fare beating, resisting arrest, disorderly conduct and disrupting government administration.
 
The NYPD did not immediately respond to a request for comment.
 
Lawson was hospitalized for his head wound, as was the officer, who reportedly had hand and wrist injuries.
 
Anas said police frequently harass young people in the station “for no reason.” She tried to get the offending officer’s badge number, but was rebuffed by officers who ordered bystanders to clear the station.
 
“It’s just so jarring,” Anas said, remembering almost breaking down as she made the short walk home. “It was horrible. I couldn’t believe my eyes.”

PATCO and SEPTA come close to fare-sharing agreement

More freedom may come to the PATCO fare card as early as 2016.

PATCO and SEPTA are closing in on a deal that would kick-off the first phase of a larger fare-sharing system, officials said. 

Officials from both transit agencies confirmed Thursday that SEPTA would accept the PATCO Freedom Card on all of its transit lines, sometime in 2016, when it unveils its new payment system, SEPTA Key.

American Airlines passengers suffered a rough arrival after a bus slammed into their plane at Hartsfield–Jackson Atlanta International Airport on Friday night.

The bus struck Flight 232 as it waited at a gate shortly after landing in Atlanta, Georgia, from Dallas-Fort Worth, Texas, according to American Airlines spokesperson Liz Landau.

If express service was never eliminated in the Bronx.....

Okay, as a Yonkers commuter, I always take the Bee-Line bus to Woodlawn and 242 Street. The thing is, there is NO express service. The (1) always runs local, why was there never a express service for the (1)? Bronx residents would love to catch a (1) express train to Manhattan.

And the (4)? Why is there no (8) express train for it? The middle track isnt being used for anything unless theres a G.O.

Track connections between separate lines and divisions

I've always been curious, where are there track connections (other than the obvious Queensboro Plaza) between the A and B divisions, the Flushing Line and the rest of the A division, and between the subway system and other railroads? Does anyone have a list or a map of these?

Okay, in case a blackout were to happen on the subway, and let's say a (Q) gets stuck in the middle of the tunnel, does everything in the (Q) train shut off (Thinking if it as it uses R160's) like the FINDS, lights, Speaker System, etc or does the train just flickr it's lights and turns on to backup power? Can the train still move if it has backup power?

WMATA to get more Xcelsior hybrids(and CNGs)

Just read on another site that WMATA is going to head back over to New Flyer once they are done receiving the 8000s Nabi BRT 42 hybrids. Apparently, they are also going to receive about 20 or so XDE60 Xcelsior artics as well to replace their 2002 Neoplan AN460 artics and XN40s to replace their C40LFs and Orion V diesels. I just knew this was going to happen since Nabi is going out of business next year. Reminds me of Orion's closure nearly 2 years ago. 

DD50 series question.

Is there a reason why the DD50 EGR and DD50G EGR both got discontinued a few years ago? I figured that the EGR component was an epa add on to reduce toxic emissions, so why discontinue it? The Series 50 only been in production for about 12 years or so (1993-2005).

Which subway cars you wish that never got retired?

Which subway cars that are going to be retire soon or already retired that you wish they had an upgrade just like the R62s and have a longer lifespan? I am hoping the (MTA) will upgrade the R32, R38, R46 & R68 with new feature such as  LED sign,  new brakes, better A/C and Automated Announcements.
